Key Responsibilities
Utilize behavior change solutions and coaching techniques to
enhance patient outcomes
Conduct video consultations and develop personalized care plans
using motivational interviewing and SMART goals
Collaborate with interdisciplinary teams to ensure optimal patient
care and address any unexpected issues
Required Qualifications
NBHWC Board Certified Health and Wellness Coach
Bilingual, Spanish speaking
2+ years of experience as a health coach or wellness educator
Proficiency with virtual care technologies and data management
Understanding of motivational interviewing and behavioral change
techniques
